What You Need — Birth Registration

  • Completed application form
  • Parent/guardian valid ID (original and copy)
  • Hospital or midwife confirmation
  • Proof of address (utility bill)

What You Need — Death Registration

  • Completed application form
  • Informant valid ID
  • Medical certificate of cause of death
  • Proof of address of the deceased

How the Process Works

  1. Visit the Civil Registry or submit your application online
  2. Present your documents — our team will verify and process them
  3. Receive your certificate — officially stamped and archived for future verification
